Users must arrange for the updating of their data and must present all personal documents requested from them in this Family Health Unit, including the Citizen’s Card or Passport.
All users covered by the various health subsystems must ensure the validity of the respective cards.
In case of change of any of your data, in particular contact, you should inform the professionals of this USF for information to be updated.
